ABOUT ALTERNATIVES IN MOTION
Getting You in a Chair
In a nutshell, here's the procedure for getting the equipment you need:
- There must be a need—not just a want. You must have a written and signed medical script provided from your medical provider.
- Take the script to the vendor of your choice. (We have a list of vendors you can choose from. If you prefer, just call us—616.493.2620 or toll free at 877.468.9335.)
- The vendor will help determine the specific type of equipment you need—size, seating requirements, etc.
- The vendor will also determine what your insurance company or other available funding source will or won't pay. If you are denied funding through these other sources, the vendor will secure a written denial from them.
- The vendor will either help you fill out an Alternatives in Motion application, or you can print out the appropriate form (see below) and take it to your vendor.
- Application Completed by Applicant (48 KB PDF) – Use this form if the applicant will be completing the form on his or her own behalf.

- Once the application is complete, that form plus the following information must be sent to Alternatives in Motion:
- Medical script
- Copies of insurance or other funding sources denials
- Copy of your 1040
- As soon as all documentation is received, the fun begins—securing the funds necessary to purchase your equipment.
